Why Edibles Hit Harder and Last Longer: First-Pass Liver Metabolism and the Creation of 11-Hydroxy-THC
Smoking and eating the same amount of THC do not produce the same effect. The reason is not dose. It is route of administration and what your liver does to THC before it ever reaches your brain.
The Two Different Paths
When you inhale delta-9-THC, it enters alveoli in the lungs, diffuses into oxygenated blood, travels via pulmonary vein to the heart, and is pumped directly to the brain. Onset is 2-10 minutes. Bioavailability is high because it bypasses the liver initially. The molecule that reaches CB1 receptors is primarily delta-9-THC itself.
When you ingest THC in an edible, oil, or beverage, the path is completely different. THC is lipophilic and absorbed through the intestinal wall into the mesenteric veins that drain into the portal vein. The portal vein carries nutrient-rich, oxygen-poor blood directly to the liver. This is called first-pass metabolism. Before THC ever reaches systemic circulation and your brain, the liver gets first chance to metabolize it.
CYP450-Mediated Hydroxylation in Liver Microsomes
Inside hepatocytes in liver microsomes, cytochrome P450 enzymes, primarily CYP2C9 and CYP3A4, perform Phase I oxidation. The reaction is: delta-9-THC + O2 + NADPH → 11-Hydroxy-THC. A hydroxyl group (-OH) is added at the carbon-11 position.
The parent compound delta-9-THC is C21H30O2. The metabolite 11-Hydroxy-THC is C21H30O3. That single oxygen changes everything.
11-Hydroxy-THC is not an inactive waste product. It is an active metabolite. It is more polar than delta-9-THC, which allows it to cross the blood-brain barrier more readily. In animal models and human reports, it is estimated to be 2-4x more potent at CB1 and produces a more body-heavy, longer-lasting psychoactive effect.
After 11-Hydroxy-THC is formed, it enters systemic circulation via the hepatic vein to the inferior vena cava, then to the heart, then to the brain, gut, and peripheral tissue where both CB1 and CB2 receptors are located.
Key Concepts That Explain Edible Experiences
First-Pass Effect Reduces Bioavailability But Increases Potency: The liver metabolizes a substantial portion of THC before systemic circulation, which is why oral bioavailability is 4-20% compared to 10-35% inhaled. However, because what does get through is largely converted to the more potent 11-Hydroxy-THC, the subjective effect feels stronger per milligram absorbed.
Onset Variability Is Physiological: Onset is 45-120 minutes, sometimes longer. Gastric emptying rate, presence of fat in the meal, individual CYP2C9 genetics, and whether you have a full or empty stomach all alter how quickly THC reaches the portal vein. A high-fat meal increases absorption but also delays gastric emptying.
Duration Is Extended: Delta-9-THC has a half-life of 1-2 hours inhaled. 11-Hydroxy-THC has a half-life of 6-8 hours and is then further metabolized to 11-nor-9-carboxy-THC (THC-COOH), which is inactive. This is why edibles last 6-8 hours with residual effects up to 12.
Clinical Significance for Dosing:
Because 11-OH-THC is more potent and longer lasting, dose stacking is the most common cause of overconsumption. The liver has not finished converting the first dose before a second is taken.

Always calculate total milligrams in a batch using lab-tested input: total mg = grams input x THC% x 1000 x 0.877. Divide by portions. Start with 2.5-5mg THC equivalent if inexperienced, wait a full 2 hours before considering more, and keep product clearly labeled and out of reach of children and pets.
This metabolism is also why drug testing looks for THC-COOH in urine days later. It is the final inactive metabolite after 11-Hydroxy-THC is oxidized again.
